Sotorasib (Lumakras) Dosage Guide: How to Take It Safely for NSCLC and mCRC

Sotorasib is an oral targeted anticancer drug developed by Amgen, a small molecule inhibitor specifically targeting the KRAS G12C mutation.

Indications for Sotorasib

Sotorasib has been approved by the FDA for the treatment of:

1. Non-Small Cell Lung Cancer (NSCLC)

Sotorasib is used as monotherapy in adult patients with locally advanced or metastatic NSCLC harboring the KRAS G12C mutation, in patients who have received at least one prior systemic therapy.

2. Metastatic Colorectal Cancer (mCRC)

Sotorasib is used in combination with panitumumab in adult patients with metastatic colorectal cancer harboring the KRAS G12C mutation, in patients who have received prior chemotherapy (chemotherapy based on fluoropyrimidine, oxaliplatin, and irinotecan).

Mechanism of Action of Sotorasib

Sotorasib works by targeting a specific cancer mutation called KRASG12C, which drives tumor growth. Its mechanism of action involves permanently binding to this mutated protein, locking it in an "off" state, preventing it from sending proliferation signals to cancer cells. This stops tumor growth and leads to cancer cell death, but only affects cells with this specific mutation; healthy cells are unaffected. In some colorectal cancers, combining sotorasib with drugs that block EGFR (another cancer pathway) is more effective than either treatment alone.

Dosage and Administration of Sotorasib

(I) Precautions Before Use

Before using sotorasib, please inform your healthcare provider of all your health conditions, including:

Liver problems: If you have any liver problems, please inform your healthcare provider.

Lung problems: If you have any lung or respiratory problems other than lung cancer, please also inform your healthcare provider in advance.

Pregnant or planning to become pregnant: It is unclear whether sotorasib will harm an unborn baby.

Breastfeeding or planning to breastfeed: It is unclear whether sotorasib passes into breast milk. Do not breastfeed during treatment with sotorasib and for one week after the last dose.

(II) Standard Dosage

1. Standard Dosage for NSCLC/mCRC

960 mg orally once daily at the same time each day.

2. Precautions for taking the medication

(1) Sotorasib can be taken before or after meals.

(2) Swallow the tablet whole. Do not chew, crush, or break the tablet in half.

(3) For colorectal cancer, you will also receive panitumumab via an intravenous drip in your arm, administered by a healthcare professional. You will need to take sotorasib before receiving your first dose of panitumumab. If sotorasib treatment is temporarily or permanently discontinued, healthcare professionals will also temporarily or permanently discontinue panitumumab treatment.

3. Handling if unable to swallow whole tablets

(1) Place the prescribed dose of sotorasib in a 4-ounce (120 ml) glass of non-carbonated room temperature water (do not crush the tablet). Do not use other liquids. Stir or shake the glass for about 3 minutes, until the tablet breaks into small pieces (the tablet will not completely dissolve). The mixture may be pale yellow to bright yellow in color.

(2) Drink the sotorasib and water mixture immediately or within 2 hours of preparation. Do not chew the tablet pieces.

(3) Rinse the glass with an additional 4 ounces (120 ml) of water and drink it to ensure you have taken the full dose of sotorasib.

(4) If you do not drink the mixture immediately, stir or shake it again before drinking.

4. Management of Missed Dosage or Vomiting

Vomiting: If you vomit after taking Sotorasib, do not take an extra dose. Take the next dose at your usual time the following day.

Missed Dosage: If you miss a dose of Sotorasib, take it as soon as you remember. If more than 6 hours have passed, do not take the missed dose; take the next dose at your usual time the following day. Do not take two doses at the same time to make up for a missed dose.

Side Effects of Sotorasib

(I) Serious Side Effects

Sotorasib may cause serious side effects, including:

Liver problems

Yellowing of the skin or whites of the eyes (jaundice), darkening or "tea-colored" urine, lighter stool color, fatigue or weakness, nausea or vomiting, bleeding or bruising, loss of appetite, pain, aches, or tenderness on the right side of the abdomen.

Lung or Breathing Problems

Sotorasib can cause lung inflammation and may even lead to death. If you experience new or worsening difficulty breathing, cough, or fever, inform your healthcare provider immediately or seek emergency medical help.

(II) Common Side Effects

Common side effects of monotherapy for non-small cell lung cancer:

Diarrhea, muscle or bone pain, nausea, fatigue, cough, abnormal blood test results, etc.

Common side effects when used in combination with panitumumab for colorectal cancer:

Rash, dry skin, diarrhea, mouth ulcers, fatigue, muscle and bone pain, abnormal blood test results, etc.

Drug Interactions of Sotorasib

Sotorasib may affect how some other drugs work, and some other drugs may also affect how sotorasib works. Inform your healthcare provider about all the medications you are taking, including prescription drugs, over-the-counter drugs, vitamins, dietary supplements, and herbal supplements. In particular, inform your healthcare provider whether you are taking antacids, including proton pump inhibitors (PPIs) or H2 receptor antagonists, while using Sotorasib. If unsure, consult your healthcare provider.

Store Sotorasib at room temperature between 20°C and 25°C. The bottle has a child-proof cap. Keep out of reach of children.
