Developed by AstraZeneca in the UK, selumetinib was officially approved for marketing in China in May 2023 and is included in China's national medical insurance program. Multiple versions are available on the market.
Selumetinib's Efficacy and Functions
Based on precise blocking of MEK1/2, selumetinib effectively interrupts the MAPK signaling pathway in tumor cells, thereby significantly inhibiting tumor growth, reducing tumor size, and controlling the disease.
Selumetinib's Indications
1. Indications
Symptomatic, inoperable plexiform neurofibromas (PN) associated with neurofibromatosis type 1 (NF1).
2. Indication Population
Children aged 2 years and older with clinically confirmed unresectable tumors and significant complications (such as pain, functional impairment, or disfigurement).
Selumetinib's Efficacy
Selumetinib is indicated for children aged 2 years and older with neurofibromatosis type 1 (NF1) and symptomatic, inoperable plexiform neurofibromas (PN). Many children treated with selumetinib experienced significant tumor shrinkage. Patients suffering from long-term, severe pain and significantly impaired function also showed marked improvement, resulting in a noticeable increase in quality of life.
Precautions for Selumetinib:
1. Cardiomyopathy
May manifest as a decrease in left ventricular ejection fraction. Regular echocardiographic monitoring at baseline and during treatment is recommended.
2. Visual Impairment
Including retinal vein occlusion and retinal pigment epithelium detachment. Regular ophthalmological examinations should be performed before and during treatment.
3. Gastrointestinal Toxicity
Such as colitis, intestinal perforation, etc. If diarrhea occurs, antidiarrheal treatment should be initiated promptly and fluid replacement should be provided.
4. Skin Toxicity
Severe adverse reactions may include swelling and desquamation of the hands and feet. Monitor for severe rashes and adjust the selumetinib dosage according to severity.
5. Elevated Creatine Phosphokinase (CPK)
Be alert for the possibility of rhabdomyolysis.
6. Vitamin E and Bleeding Risk
Avoid supplementing with vitamin E beyond the safe daily intake limit; monitor for bleeding when used concurrently with vitamin K antagonists or antiplatelet drugs, and increase the frequency of INR monitoring.
7. Embryo-Fetal Toxicity
Women of childbearing age should use appropriate contraception.
Special Populations for Selmetinib Use
1. Pregnant Women
However, given its potential harm to the fetus, its use should be avoided in pregnant women.
2. Breastfeeding Women
Currently, there is no data indicating whether selmetinib enters breast milk; it is recommended to discontinue breastfeeding during treatment and for one week after the last dose.
3. Men and Women of Childbearing Age
Given its effects on the sexual function of minors, effective contraception should be used during treatment and for one week after discontinuation.
Patients using selmetinib should strictly follow their doctor's instructions regarding dosage and administration, and avoid taking it concurrently with other drugs that may interact with it. Regularly monitor liver function and lung condition to prevent possible serious adverse reactions.
