Ivosidenib Conventional Dosage for Adult Acute Myeloid Leukemia
1. Indications
(1) Newly diagnosed acute myeloid leukemia (AML):
For patients aged 75 years or older, or those with comorbidities precluding intensive induction chemotherapy.
Confirmation of susceptible IDH1 mutation using an FDA-approved test is required.
May be used as monotherapy or in combination with azacitidine.
(2) Relapsed/refractory acute myeloid leukemia (AML):
Confirmation of susceptible IDH1 mutation using an FDA-approved test is required.
2. Recommended Dosage
500 mg once daily, continued until disease progression or unacceptable toxicity.
3. Important Note
In the absence of disease progression or unacceptable toxicity, treatment should be continued for at least 6 months to allow for clinical response.
4. Combination Regimen
When co‑administered with azacitidine, ivosidenib should be started on Day 1 of Cycle 1.

1. Indication

Important Administration Notes for Ivosidenib
1. Patient Screening
(1) For AML patients, IDH1 mutation must be confirmed by blood/bone marrow testing; patients without mutation at initial diagnosis should be retested at relapse (mutations may emerge de novo).
(2) For cholangiocarcinoma patients, IDH1 mutation testing is required.
2. Administration Method
Swallow tablets whole; do not break, crush, or chew. Take at the same time each day, with or without food, but avoid high‑fat meals (may increase blood concentrations).
3. Vomiting Management
If vomiting occurs, do not take a replacement dose; resume the next scheduled dose as planned.
4. Missed Dose Management
If it is more than 12 hours until the next scheduled dose, the missed dose may be taken; if less than 12 hours, skip the missed dose.
