Special Population Considerations for Baricitinib Use

Key Precautionary Notes Before Use

Baricitinib must be prescribed and supervised by a physician experienced in the management of rheumatoid arthritis.

Strict contraindications: Hypersensitivity to the active ingredient or any excipient, and pregnancy.

Severe hepatic or renal impairment: Avoid use.

Mild to moderate hepatic or renal impairment: Dosage adjustment may be required.

Breastfeeding: Suspend breastfeeding during treatment and for 4 days after the last dose.

Elderly patients: Close monitoring of renal function is recommended.

Drug interactions: Inform your physician of all concurrent medications to avoid potential interactions.

Strict Contraindications

The following two groups are strictly prohibited from using baricitinib:

1.Patients with known hypersensitivity to baricitinib or any component of the formulation.

2.Pregnant women.

Use with Caution

Four high-risk groups require special attention:

Severe hepatic impairment: Not recommended.

Severe renal impairment: Not recommended if creatinine clearance is <30 mL/min.

Mild to moderate hepatic impairment: No dosage adjustment needed.

Renal impairment:

Mild: No dosage adjustment needed.

Moderate: 2 mg once daily (half the standard dose).

Severe: 1 mg once daily (one-quarter of the standard dose).

Special Population Considerations

Breastfeeding Women

To avoid transfer of the drug to infants via breast milk, breastfeeding should be suspended during treatment and for 4 days after the last dose (equivalent to 5–6 drug half-lives for complete elimination).

Elderly Patients

Age-related decline in renal function may affect drug clearance. Special attention should be paid to dosage adjustment, and regular monitoring of renal function is recommended.

Drug Interactions

Before initiating baricitinib, inform your physician of all concurrent medications, along with a complete medical history and treatment plan, to allow for evaluation of potential drug-drug interactions.
