Sotorasib is indicated for patients with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have received at least one prior systemic therapy, as detected by an FDA-approved test; and in combination with Panitumumab for KRAS G12C-mutated metastatic colorectal cancer.

Sotorasib Usual Adult Dose

960 mg orally once daily. Continue until disease progression or unacceptable toxicity.

Sotorasib Administration Guide

1. Administration Requirements

Take Sotorasib at the same time each day, with or without food; swallow tablets whole, do not chew, crush, or split.

2. Alternative for Patients with Swallowing Difficulties

Step 1: Disperse Sotorasib in 120 mL (4 ounces) of non-carbonated, room-temperature water (do not use other liquids). Stir until the tablet disperses into small particles (the tablet will not completely dissolve). Drink immediately or within 2 hours; the mixture will appear light yellow to bright yellow.

Step 2: Swallow the Sotorasib mixture without chewing the particles. After taking, rinse the container with 120 mL of water and drink the rinse.

If not consumed immediately, stir the Sotorasib mixture again to ensure dispersion of the tablet particles.

3. Missed Dose

If a dose is missed by less than 6 hours, take it as soon as possible. If a dose is missed by more than 6 hours, skip the missed dose and take the next dose at the regularly scheduled time the next day. Do not double the dose to make up for the missed dose (double dose is prohibited).

4. Vomiting

If vomiting occurs after taking the dose, do not take an additional dose; take the next dose at the scheduled time the next day (no need to compensate).

Management of Hepatotoxicity

1. Grade 2 AST or ALT elevation with symptoms, or Grade 3-4 AST/ALT elevation: Withhold Sotorasib until recovery to Grade 1 or below or to baseline; upon resumption, reduce the dose by one level. First dose reduction: 480 mg orally once daily; second dose reduction: 240 mg orally once daily.

2. AST or ALT > 3 times the upper limit of normal (ULN) with total bilirubin > 2 times ULN, without other identifiable cause: Permanently discontinue Sotorasib.
