Overview of Eltrombopag

Eltrombopag is an oral non-peptide thrombopoietin receptor agonist (TPO-RA). It increases platelet production by activating the thrombopoietin receptor and promoting the proliferation and differentiation of bone marrow megakaryocytes. It is primarily indicated for the treatment of adult and pediatric patients (12 years of age and older) with chronic immune thrombocytopenia (ITP) who have an inadequate response to therapies such as corticosteroids or intravenous immunoglobulin.

Drug Interactions

Drugs containing calcium, iron, or aluminum (e.g., antacids, dairy products): May significantly reduce the absorption of eltrombopag. Administration should be separated by at least 4 hours.

Strong CYP3A4 inducers/inhibitors (e.g., rifampicin, ketoconazole): May affect plasma drug concentrations, requiring dose adjustment.

Anticoagulant/antiplatelet drugs (e.g., warfarin): May increase the risk of bleeding.

Special Population Precautions

Pregnant Women

Use only if the potential benefit justifies the potential risk to the fetus. Animal studies have demonstrated fetal toxicity.

Lactating Women

Breastfeeding should be avoided, as the drug may be excreted in human milk.

Patients with Hepatic Impairment

Patients with moderate to severe hepatic disease require a reduced starting dose (25 mg) and close monitoring.

Pediatric Patients

Dosing must be adjusted based on body weight; indicated for patients 1 year of age and older.

General Precautions

Monitoring Requirements

Platelet counts: Weekly monitoring during the initial treatment phase, then monthly once stable.

Liver function: Monitoring every 2 weeks.

Management of Missed Doses

If a dose is missed, take the usual dose at the next scheduled time. Do not double the dose to make up for the missed dose.

Discontinuation Principles

Dosing should be tapered gradually. Abrupt discontinuation may cause a sudden drop in platelet counts.
