Dosage and Administration

Bedaquiline Fumarate Tablets:Swallow whole with water and take with food.
Recommended Dosage
Weeks 1–2: 400 mg orally once daily
Weeks 3–24: 200 mg 3 times weekly (with at least 48 hours between doses)
Total Duration of Treatment
The total treatment duration is 24 weeks. Data for longer‑term use are limited.Extended treatment may be considered only on an individual basis with close safety monitoring in patients with extensively drug‑resistant disease when deemed necessary for cure.
Overdose Management
There is no specific experience with acute overdose.In case of intentional or accidental overdose, general supportive measures should be instituted, including monitoring vital signs and ECG (QT interval).
Unabsorbed bedaquiline may be removed using activated charcoal.Dialysis is unlikely to significantly eliminate plasma bedaquiline due to its high protein binding.
Missed Dose Management
Weeks 1–2: If a dose is missed, do not make up the missed dose; resume the regular schedule.
Week 3 onwards: If a 200 mg dose is missed, take the missed dose as soon as remembered, then continue the 3‑times‑weekly regimen.
Bedaquiline must be used in combination with other prescribed antimycobacterial drugs, and full course adherence is required.Missing doses or incomplete therapy may reduce efficacy, increase risk of mycobacterial resistance, and limit future treatment options.
Contraindications
Bedaquiline is a prescription drug and must be used strictly as directed.The following groups are generally contraindicated:
Patients with hypersensitivity to bedaquiline or any component of the product
Patients with latent tuberculosis infection caused by Mycobacterium tuberculosis
Patients with drug‑susceptible tuberculosis
