Dosing Guidelines for Alpelisib

Patient Selection

Alpelisib is indicated for patients with hormone receptor‑positive (HR+), human epidermal growth factor receptor 2‑negative (HER2‑) advanced or metastatic breast cancer, selected based on the presence of one or more PIK3CA mutations in tumor tissue or plasma specimens. If no mutation is detected in plasma, tumor tissue should be tested.

Dosage

The recommended dose of alpelisib is 300 mg (two 150 mg film‑coated tablets) taken orally once daily with food.Treatment should continue until disease progression or unacceptable toxicity.

Administration

Patients should take alpelisib at approximately the same time each day.Alpelisib tablets should be swallowed whole (do not chew, crush, or split before swallowing). Do not take tablets that are broken, cracked, or otherwise damaged.

Missed Dose

If a dose of alpelisib is missed, it may be taken with food within 9 hours of the usual time. If more than 9 hours have passed, the dose should be skipped entirely, and the next dose should be taken at the normal time.

Vomiting

If vomiting occurs after taking alpelisib, an additional dose should not be taken that day. Dosing should resume at the usual time the next day.
