Cabozantinib recommended dosage is closely related to the treated disease and patient age; typically, once daily, with specific dosage strictly following physician's instructions.
Adult Usual Dosage of Cabozantinib
1. Thyroid Cancer
(1) Indications
For progressive metastatic medullary thyroid cancer (MTC).
For the treatment of adult patients with locally advanced or metastatic differentiated thyroid cancer (DTC) who have progressed after prior VEGFR-targeted therapy and are radioactive iodine-refractory or unsuitable.
(2) Dosage and Administration
Tablet formulation (DTC): patients with body surface area (BSA) ≥1.2 m² receive 60 mg once daily until disease progression or unacceptable toxicity.
(3) Note
Discontinue treatment at least 3 weeks before scheduled surgery (including dental procedures).
2. Renal Cell Carcinoma (RCC)
(1) Indications
For patients with advanced renal cell carcinoma (RCC).
In combination with nivolumab for first-line treatment of advanced RCC.
(2) Recommended Dosage
Tablet formulation, cabozantinib monotherapy: 60 mg once daily orally until the patient no longer derives clinical benefit or unacceptable toxicity occurs.
Cabozantinib in combination with nivolumab: 40 mg once daily orally until disease progression or unacceptable toxicity.
(3) Note
When cabozantinib is used in combination with nivolumab, refer to the prescribing information for nivolumab. Tablets and capsules are not interchangeable.

3. Hepatocellular Carcinoma (HCC)
(1) Indications
For patients with hepatocellular carcinoma (HCC) previously treated with sorafenib.
(2) Dosage and Administration
Tablet formulation: 60 mg once daily orally until the patient no longer derives clinical benefit or unacceptable toxicity occurs.
(3) Note
Discontinue treatment at least 3 weeks before scheduled surgery (including dental procedures).
