Avatrombopag (Doptelet) increases platelet production by mimicking the action of endogenous thrombopoietin (TPO), activating TPO receptors, and promoting the proliferation and differentiation of megakaryocytes in bone marrow progenitor cells.
Avatrombopag (Doptelet) Indications
Avatrombopag (Doptelet) is used to treat thrombocytopenia (low platelet count) in the following populations:
(1) Adult patients with chronic liver disease scheduled for dental or medical surgery.
(2) Adult patients with chronic immune thrombocytopenic purpura (ITP) who have not responded well to other treatments, or children with chronic/persistent immune thrombocytopenic purpura.
Avatrombopag (Doptelet) Medication Management
1. Medication Management
(1) Chronic Liver Disease (CLD)
Usually taken once daily for 5 consecutive days, starting 10-13 days before the planned procedure.
The procedure is usually scheduled 5-8 days after the last dose.
Strictly follow your doctor's instructions regarding medication and procedure timing.
Your platelet count will be checked by healthcare professionals before treatment and on the day of the scheduled procedure.
If you miss a dose before the scheduled procedure, please contact your healthcare professional for further medication guidance.
(2) Immune Thrombocytopenic Pulmonary Disease (ITP)
Avatrombopag (Doptelet) is usually taken once daily.
Your platelet count will be checked by healthcare professionals before, during, and for at least 4 weeks after discontinuation of the medication.
Note: Dosage or frequency may be adjusted based on platelet count.
2. Dosage for Children
(1) Open the avatrombopag (Doptelet) capsule and sprinkle the contents onto a small amount of soft food or liquid.
(2) Mix the capsule with the soft food or liquid and drink immediately.
(3) Do not swallow the capsule whole or the empty capsule shell.
(4) Do not chew or crush oral granules.
(5) Strictly follow the instructions of the child's healthcare provider, including dosage, start time, and possible dosage adjustments; take with food.
Avatrombopag (Doptelet) Standard Dosage
1. Chronic Liver Disease (Adult Patients)
(1) Platelet count below 40 × 10⁹/L: 60 mg orally daily for 5 consecutive days, starting 10-13 days before the planned procedure.
(2) 40 × 10⁹/L to below 50 × 10⁹/L: 40 mg orally daily for 5 consecutive days, starting 10-13 days before the planned procedure.
2. Chronic Immune Thrombocytopenic-Please Disease (Adult Patients and Children 6 Years and Older)
(1) 20 mg orally daily.
(2) Adjust dosage or frequency to maintain platelet count ≥ 50 × 10⁹/L.
(3) The maximum daily dose should not exceed 40 mg.
3. Chronic Immune Thrombocytopenic Pulmonary Disease (Children aged 1-6 years)
(1) 10 mg orally daily (1 avatrombopag powder capsule).
(2) Adjust the dosage or frequency of administration to maintain a platelet count ≥50 × 10⁹/L.
(3) The maximum daily dose should not exceed 20 mg.
Avatrombopag (Doptelet) Side Effects
1. Common Side Effects
(1) When used in adult patients with chronic liver disease (CLD):
Fever, headache, abdominal pain, fatigue, nausea, swelling of the hands and feet.
(2) When used in adult patients with immune thrombocytopenic purpura (ITP):
Headache, joint pain, fatigue, bleeding gums, bruising, purple or red spots on the skin, nosebleeds, runny nose, upper respiratory tract infection.
(3) For use in children with immune thrombocytopenic purpura (ITP) (tablets or powder):
Viral infection, fever, runny nose, sore mouth or throat, cough.
2. Serious Side Effects
(1) Allergic Reactions
If signs of an allergic reaction to avatrombopag (Doptelet) occur, seek immediate medical attention:
Hives, difficulty breathing, swelling of the face, lips, tongue, or throat.
(2) Serious Side Effects
Blood clots may occur during the use of avatrombopag (Doptelet). If the following symptoms occur, contact your doctor immediately or seek emergency medical help:
Chest pain, shortness of breath, rapid heartbeat, pain, swelling, or redness in one or both legs, abdominal pain or tenderness, sudden fever or chills, yellowing of the skin or eyes (jaundice), bloody or tarry stools, coughing up blood or vomiting coffee-ground-like vomit.
Doptelet (Avatrombopag) Missed Dosage Management
1. Chronic Immune Thrombocytopenic-Pulmonary Disease (CITPD)
If you remember missing a doptelet, take it immediately. Take the next dose at the usual time, maintaining a once-daily dosing regimen. Do not double the dose.
2. Before Planned Medical Surgery
If you miss a doptelet, contact your doctor for guidance.
Doptelet (Avatrombopag) Overdose
If you overdose on doptelet (Avatrombopag) tablets or powder, contact healthcare professionals immediately.
Doptelet (Avatrombopag) Drug Interactions
1. Other medications may interact with doptelet, including prescription drugs, over-the-counter drugs, vitamins, and herbal supplements. Inform your doctor about all medications you are currently using, as well as any new or discontinued medications.
2. Drugs that interact with avatrombopag (Doptelet) include:
Dual moderate or strong inhibitors of cytochrome P450 (CYP)2C9 and CYP3A;
Dual moderate or strong inducers of cytochrome P450 (CYP)2C9 and CYP3A4.
Avatrombopag (Doptelet) Storage Instructions
Avatrombopag (Doptelet) should be stored at room temperature, avoiding moisture and high temperatures. Before administration, tablets should be stored in their aluminum-plastic blister packs, and powders or capsules should be stored in their original packaging.
